After submitting your application, you will receive an email from Adobe Acrobat within 5 to 10 business days. This email will contain a link to an online PDF form that must be completed in order to volunteer or intern with PSS.

For applicants 17 years old or younger:
You will receive the same email, but after you sign the form, another email will be sent to your Parent/Guardian at the email address you provided for them. This email, also from Adobe Acrobat, will contain a link for them to sign and date the form online. Their signature is required for you to participate.

APPLICANT'S STATEMENT

I affirm that the information provided on this application (and any accompanying resume) is true and complete to the best of my knowledge. I understand that any falsified information or significant omissions may disqualify me from further consideration for volunteer employment and could be grounds for dismissal if discovered at a later date.

I acknowledge that if a volunteer/intern employment offer is extended to me and accepted, I will fully adhere to the policies, rules, and regulations of volunteer employment set by the Employer. I further understand that neither these policies, rules, and regulations nor any statements made during the interview process constitute the terms of an implied employment contract. I also understand that any volunteer employment offered is at will, meaning it is for an indefinite duration and may be terminated by either the Employer or myself at any time.

As part of PSS's prospective volunteer/intern screening process to ensure its constituents receive the highest level of professional and competent service, I understand that background checks and possible fingerprinting may be conducted on viable applicants. This process may include obtaining a credit report containing financial or other personal information. By signing below, I authorize PSS to obtain a background check and/or credit report as part of this screening process. I also understand that PSS complies with the Fair Credit Reporting Act, which provides consumers with rights regarding consumer reports and places specific obligations on employers using such reports.

I grant full permission to PSS and its agents to use photographs, portraits, films, videos, and quotations of or by me for legitimate accounts, advertisements, and promotions of its programs and services in printed materials and online without further notice or compensation to me.
